Why Paperwork?

Do you feel like you are constantly filling out forms? The reason is that you probably are.
What you do not realize is that we are not giving you these forms to “be mean” but to protect you each and every visit. Paperwork is our written approval to proceed with your visit, bill insurance on your behalf, and know how and where we can contact you.

We have all become so “tech savvy” that there are many ways to communicate to a person. We can call your home phone, your cell phone, or your work phone. We can send you an email or instant message. We can even use “snail mail” and mail something to your home. Along with our many options to communicate, it is also common for many people to change their phone numbers, email addresses, home address, and even their significant others. For these reasons, we work very diligently to keep our records as up to date as possible. With the HIPPA (The Health Insurance Portability and Accountability Act) laws being in place to protect you, the patient, we cannot communicate with you by any of the means above unless we have been given direct permission to do so. Medical practices can no longer assume it is ok to discuss your health, test results, patient bills, or even appointments with your spouse, child, or other friends and family without consent to do so.

So the next time you have an appointment with our office, please come in prepared to fill out paperwork by arriving 15 minutes early, always bringing your insurance card, photo ID, and current list of medications, and instead of questioning the necessity of this paperwork, be very thankful and appreciative that this office cares enough about your privacy and the ability to communicate with you, to keep our records as accurate as possible.
For all you patients who have taken advantage of our patient portal system, that is also a great way to communicate changes to your name, address, phone number and medical information between your visits at the office.

Due to our longstanding relationship with our patients, our goal is to provide you with exceptional medical care without compromising your privacy each and every time.
